Care Home Telephone Assessment – Diatetics

Overview

Using a telephone based triage system to improve timely clinical advice to care home residents.

What is it?

High demand for dietetic input in care home residents, where referrals can be lacking in detail. In this past this has meant that it was not always possible to establish priority of need, or the most appropriate mode of treatment.

Outcome

This is now an integral part of the working pattern for the core community dietetics team. Standardised assessment paperwork and pathways are in place, and processes allowing for accurate recording. The Yorkshire & Humber AHSN are assisting with developing this further for wider adoption and spread

Case Study Templates

Overview

Please help us by sharing a case study of something (post IP if relevant) of something your organisation or you as an Innovation Champion have been working on, proud of, done, developed, adopted from elsewhere and what the outcome has been? Literally half to a side of A4 with a picture. We can do the rest.
Alternatively you may want to write up a bit about yourself and what you've been up to as an Innovation Champion

What is it?

Please look at the completed case studies for a rough idea of how much to write. Not all of the questions may be appropriate and you may even have some points you want to add - that's fine. Whatever you feel is best to tell the story you want to.

Outcome

Please send your case study to Fabian (details in the template) who will do the final formatting. Remember to include a picture whether it is of yourself, innovation, team or whatever!
